Position Description:
The Disease Management Care Coordinator (DMCC) makes outbound calls to assess the members current health status and understanding of their chronic condition.
The DMCC identifies gaps or barriers that would prevent the member from following through with a recommended treatment plan consistent with optimal care for specified disease conditions.
The DMCC provides education to assist with self-management, provides information on network access, and identifies and implements interventions and resources that address and close gaps in care.

Major Responsibilities:
Within designated timeframes, contact members are identified for the specific disease management program and promote program enrollment.
Following enrollment, conduct an initial assessment of the members clinical, behavioral, social, environmental and health-related financial circumstances or needs that may impact compliance with the optimal treatment plan.
Following assessment, prioritize care needs and develop a plan of care to address gaps and or barriers, as well as to track the members health status and progress in achieving clinical and personal goals.
Promote the patient/provider relationship to support compliance with nationally established guidelines of care.
Conduct follow-up assessments or interventions based on members needs.
Conduct PRN calls when key gaps are identified that require additional nursing follow-up.
Engage members, family, caregivers, and health care providers to assure that a well-coordinated longitudinal treatment plan is established.
Employ a holistic approach to patient care and integrate the members life and motivational goals into the care plan Identify lifestyle or behavior modification needs, direct members to available resources and reinforce behavior modification changes.
Work to facilitate patient compliance and ensure continuity of care.
Monitor and evaluate members' response to treatment and work with providers and Optum medical directors to revise care plans as needed.
Provide education, information, direction and support related to the care goals of members.
Coordinates acquisition and proper use of medical equipment, initially and on an ongoing basis.
Assure that member receives appropriate fulfillment materials for condition.
Document assessments, interventions and follow-up activities in the Care Coordination System
Work with supervisors to regularly assess the effectiveness and quality of services provided to members by analyzing outcomes (clinical, process, member satisfaction and financial).
Maintain a focus on timely, quality customer service.
Maintain a focus on call efficiency and operational metrics.
Follow direction from supervisors regarding policy & procedures, training, or management of the member As requested, participate in demonstrations to customers or other parties interested in the disease management program
Obtain certification in disease management if requested and supported client.

Must be a LPN, licensed Florida
BSN preferred
Active Professional Nursing License required Strongly preferred
Training, certification, education and/or work experience in cardiac and diabetes conditions managing with patients in case or disease management programs
Minimum of 3 to 5 years of clinical experience (medical/surgical, coronary care, intensive care, community home health nursing, home health care, behavioral health) and / or 3 to 5 years in a managed care environment with experience in case or disease management
